Treatment of fibroids for Successful IVF
What is a fibroid?
Uterine fibroids are muscular tumours that form in the wall of the womb. Mostly fibroids are non-cancerous and the most common benign tumour of female genital tract.Older women are more likely to have fibroids. It is estimated that more than 70% of the women in reproductive age may have fibroid. The lifetime risk of fibroid is over 60% after 45 yrs.
Most of the women with fibroid has no symptom.Abdominal pain, heavy flow and pain during periods are the common symptoms.Sometimes they can lead to infertility or miscarriage. The symptoms, that the fibroid can cause, depend upon their location and size.
Fibroids can be a single growth or there can be multiple small tumors in the uterus. The size of a fibroid can be as one inches to 8 inches or even more. The larger the fibroid, the more problems it is likely to cause in the uterus regarding a woman’s ability to conceive. It is not only the size of the fibroid that can affect a woman’s ability to achieve pregnancy, but also the localization.
Fibroids are classified in three major categories based on their precise locations:
- Submucosal fibroids: These are the types of fibroids that grow in the uterine cavity
- Intramural fibroids: These are the fibroids that grow within the wall of the uterus
- Subserosal fibroids: These are the fibroids that grow on the outside of the uterus.
How Do Fibroids Cause Infertility?
Fibroids located above the cervix can change its position and therefore restrict passage of sperm cells that can travel through the cervix.
– Certain fibroids can block the fallopian tubes, thus restricting oocyte movement and fertilization.
– Some fibroids can restrict the blood flow into the uterine cavity, where the embryo would normally implant and generate a pregnancy.
– Fibroids can also possibly cause changes in the uterine muscle that prevents embryo movement.
How fibroid can affect result of IVF?
Not all the fibroids are known to affect the IVF success rate. Those myomas which distort the uterine cavity or those which are very large in size can lead to infertility.
Treatment of myoma before IVF
The 3 D USG is sometimes needed by IVF specialist to know the exact location of fibroid. Only the large intramural fibroids or fibroids that distort the uterine cavity can negatively affect the results of IVF and hence should be surgically removed prior to IVF treatment.
