PCOS
Polycystic Ovarian Syndrome (PCOS) occurs in 5-10% of women of reproductive age. The condition is characterized by abnormal ovarian function (irregular or absent periods, abnormal or absent ovulation and infertility), androgenicity (increased body hair or hirsutism, acne) and increased body weight/body mass index or BMI. The ovaries of women with PCOS characteristically contain multiple immature follicles.

PCOS and infertility
Women with pcos have basically problems with their ovulation.They don’t conceive as they don’t ovulate or ovulate irregularly.Some of them experience more severe anovulation and don’t get their normal periods.They need to take hormone to get their menstrual flow ( withdrawl bleeding ).
Sometimes,there are other associated factors like male or tubal factor that may contribute to infertility.
Diagnosis
It often takes a while for women with PCOS to get a firm diagnosis, as the condition can mimic other problems.
With early diagnosis, the symptoms of PCOS can be treated early. This reduces the risk of complications and time taken if one decides to become pregnant.
The doctor needs a proper history and examination,certain blood tests and ultrasound scan to make the diagnosis.
Treatment options for PCOS
The preliminary measures to improve pregnancy rates are
- Maintaining a healthy weight – even a 5-10% loss in weight has been shown to significantly increase the chance of becoming pregnant. The most important step is to have a diet which is rich in nutrient-rich foods and adequate protein and low on sugar . Avoiding junk and processed food add additional benefits.
- Exercise
- Monitoring ovulation and timing sexual intercourse around ovulation.
Other options for those who have not conceived with above-mentioned measures include further advice regarding ovulation inducing medications ( usually clomiphene ) with evaluation of other contributing factors. Clomiphene is the most commonly used fertility drug overall, and also the most commonly used treatment for women with PCOS. Many women with PCOS will conceive with Clomiphene In those with insulin resistance, taking the diabetes drug metformin can treat the insulin resistance and may help in inducing ovulation.
If Clomiphene or letrozole is not successful, the next step is gonadotropins. Gonadotropins. are made of the hormones FSH, LH, or a combination of the two.
The gonadotropins treatment cycle is often combined with an IUI (intrauterine insemination) procedure to increase the success still further. IUI involves placing specially washed semen directly into the uterus via a catheter.
Surgical treatment
Laparoscopic ovarian drilling is a surgical procedure sometimes used in the treatment and management of polycystic ovarian syndrome.Like any surgery,the procedure has risk of potential complications and that needs to be considered in decision making for the patient.
